This form requests registration information for users of the
Clinic Modeler software. Members of the United States Department of Defense (active service
members or civilian employees) are permitted free
registration (below) of the version 1.0 of the Clinic Modeler software.
Government contractors are required to purchase a license prior to
continued use beyond the initial 30 day trial period.
To continue use of the software beyond the initial trial period
requires registration of contact information below. Upon
submission of the contact information, including the software serial
number (displayed in the Trial Version window, see right) a registration
key file will be emailed to you unlocking the software for unlimited use
on that computer. The software must be registered for each
individual computer.
All other non-US Government users are required to purchase a license to the
software. Purchasing a license provides updates and patches. To purchase a license click the purchase
